For most children and young people in the hospital, the discharge process is simple and uncomplicated. For some, their needs are more complex.
Often parents and carers of children and young people with complex or exceptional healthcare needs have or will need multi-agency support for the rest of their life. This includes additional support to promote family life and ensure safety at home.
To put the right support in place, detailed planning and work with other teams and organisations are often needed. If a child or young person needs this support, an introduction to the service can be made by the Lead Consultant or another professional involved in the patient’s care. The Discharge Liaison Nurse will work with everyone necessary to help a smooth transition back home. This might include social work, education, and community-based services like physiotherapy or dietetics.
