Discharge Liaison Nurse Community Children’s Nursing Team RHSC Edinburgh

The Discharge Liaison Nurse role is to oversee and facilitate the discharge of children with particular complex health needs from RHSC to home.

The role involves working very closely with families and all members of the team around the child within the hospital and in the community.
GIRFEC is a Scottish Government led agenda that all Health Boards and Local Authorities have agreed to. This is to ensure that anyone providing support puts the child or young person, and their family at the centre. It has been put in place to allow early intervention and to work across all agencies.

Getting it Right for Every Child (GIRFEC)

In line with best practice and principles all children up to the age of 18 will have a Named Person. This is the Health Visitor for pre-school children and then transferred to a teacher in a promoted role such as guidance/year head/deputy or Head teacher from your child’s school.  Children or young people with additional support needs may have a Lead Professional.

As part of this integrated working, the Discharge Liaison role is further being developed using the Getting it Right Agenda as a standard of practice in communicating with the Named Person and Lead Professional. We will always inform you of communications required to the Named Person/Lead Professional and seek consent when it is from an external agency.
